The decision on whether or not to go throught the HAD-4-Kids^TM program should be mutually agreed upon between the child's service providing Audiologist and the parents/legal guardians. The Audiologist must agree to provide services for the child and the parent must agree to pay for the services to the audiologist and pay for the hearing aids through the HearingAidDocs^TM.  Please review the steps below for more detail.

2. Audiologist: Please send/fax/mail the following to the HearingAidDocs^TM :

• Services Agreement Form Signed by the legal guardian/parent.
• Hearing Evaluation (within last six months) signed by Audiologist
• A signed letter of Recommendation written by the Audiologist with a brief description of the diagnoses and reason for the hearing aids recommended.

PLEASE SEND ALL 3 DOCUMENTS TO US TO PURCHASE HEARING AIDS

• Once we have received the documents requested above we will send  the parents/guardians, an email confirmation so you may purchase the hearing aid(s).
• Upon completion of the previous steps and review of the information, the HearingAidDocs^TM will mail the hearing aid(s) directly to the Audiologist/Hearing Health Care Provider.
• The Audiologist will call the patient upon receipt of the hearing aid to schedule a hearing aid fitting appointment.  The parents/guardians/patient will pay the audiologist the previously agreed upon servicing fee at this appointment.
• The HearingAidDocs^TM will follow-up with the parents/guardians/patient several weeks after the fitting of the hearing aid to to check in.
